After considering several checklist type instruments, the task force selected the Mass Casualty Disaster Plan Checklist: A Template for Healthcare Facilities, developed by the Association of Professionals in Infection Control and Epidemiology (APIC) and the Center for the Study of Bioterrorism and Emerging Infections, 4 as the basis for the survey instrument.
