We report a case of a 46yr old obese male with acute respiratory distress syndrome (ARDS) secondary to COVID-19, who went on to develop Fournier's gangrene following prolonged and repeated ventilation in prone position (proning). Despite optimized positive end-expiratory pressure (20 cmH 2 O) and lungprotective ventilation, the patient had ongoing hypoxia with PaO 2 /F I O 2 ratios less than 150, and by day 4 of ICU admission was started on a 16-18 hrÁday -1 proning protocol.
