In this review we will provide an overview of the relationship between respiratory virus infection and the molecular mechanisms involved in the activation of airway infl ammation in COPD exacerbations. It has been postulated that bacterial colonization could contribute to increased susceptibility to viral infection in COPD patients for example by increasing ICAM-1 expression in bronchial epithelial cells either directly or through induced infl ammation (Sajjan et al 2006) .
