This prospective case control study was carried out on 50 patients, suffering from hypercapneic acute respiratory failure as a result of acute exacerbation of chronic obstructive pul-monary disease (COPD), receiving non invasive mechanical ventilation admitted consecutively to critical care units at Alexandria university main hospital. PS or inspiratory positive airway pressure (IPAP) was initially set at 10 cm H 2 O and then titrated up to achieve an expiratory tidal volume of 8-10 ml/kg and a respiratory rate below 25 breaths/ min to a maximum of 25 cm H 2 O depending on clinical and arterial blood gases (ABGs) response and patient tolerance.
