A point of care ultrasound ( Figure 3 Figure 4 ) on day 7 of hospitalization revealed thrombus in transit in the inferior vena cava and right atrium suggesting impending pulmonary embolism hence the patient was started on thrombolytic therapy and full dose anticoagulation with heparin drip.
