We outline the classical ethical framework that guides heart and lung transplantation and mechanical circulatory support (MCS) and discuss adaptations to this construct during different stages of the ongoing pandemic. In order to analyze how to adjust treatment strategies for advanced heart and lung disease, we propose separating the effects of the COVID-19 pandemic from a healthcare system perspective into different stages.
