Anecdotally, corticosteroids are frequently used in the setting of severe fungal pneumonia, particularly due to Histoplasmosis [11, 12] , and a small controlled trial of 55 patients supported their use in miliary tuberculosis [13] . Following the success of pre-antibiotic corticosteroids in children with meningitis [14] , Marik and colleagues [15] studied the effect of a single dose of hydrocortisone (10 mg/kg) 30 min prior to antibiotic therapy in a small randomized placebo controlled trial of 30 adult patients with severe CAP (SCAP).
