We demonstrate the power of clinical mNGS in elucidating the cause of uncommon and unexpected infections and identify a case of chronic HEV infection most likely transmitted through the transplanted lungs the patient had received 6 years prior. The case patient is a 58-year-old woman with a history of idiopathic pulmonary fibrosis status post bilateral lung transplant in 2011, migraines, hypercoagulability, and multiple sclerosis (MS) on chronic immunosuppression who was admitted to University of California, Los Angeles Medical Center in October 2016 with 8 days of fever, headache, nausea, vomiting, neck stiffness, and photophobia.
