Optimal perioperative management and risk stratification should expand beyond the well-described airway challenges and consider numerous systemic manifestations of systemic sclerosis such as pulmonary arterial hypertension, interstitial lung disease, and cardiac sequelae. Separately, pulmonary hypertension related to ILD or SScrelated cardiac involvement may increase both morbidity and mortality in the perioperative period and may be present in 8-12% of SSC patients [23, 25•] .
