We report to our knowledge, one of the first reports of COVID-19 infection in an infant with bronchopulmonary dysplasia (BPD) who developed severe acute respiratory distress syndrome (ARDS), causing bradycardic cardiac arrest and seizures. A 4-month-old male infant was born prematurely at 28 weeks gestation (corrected age is 1 month) with a very low birth weight of 1.315 kg, and was on nasal continuous positive airway pressure/highflow nasal cannula (CPAP/HFNC) for 90 days for respiratory distress syndrome at birth. His neonatal intensive care unit (NICU) admission was complicated by human metapneumovirus pneumonia and apnea at 63 days of life. He was weaned off oxygen and discharged home from the NICU 101 days after birth on multivitamins, chlorothiazide, spironolactone, albuterol, and budesonide. He had been home for 2 weeks when he developed cough, nasal congestion, and increased work of breathing 5 days before admission. He was cared for by his parents who were sick with COVID-19 symptoms. The patient presented to the emergency department with tachypnea, retractions, and hypoxemia with saturation by pulse oximetry of 77%. He developed generalized tonicclonic seizures and had a bradycardic cardiac arrest and was resuscitated. There was no history of fever, vomiting, diarrhea, rash, or decrease in oral intake.
