¶ We on the behalf of our said sovereign lord the king / and by authority of the said lettres patents & act we charge and command you / & every of you vpon the pain of. x.li. that ye personally appear before us / or before us & the other commissioners in the said lettres patents assigned / or before us / vi. v. iiij. iij. or. ji. of us at the 〈…〉 day of this present month of march / at the hour of .viij. of the clock before noon. And there certify us by your writing the ●amess and surnames of all & singular persons men & women above the age of .xv. yeres being within the said ward / & dwelling / abiding / or having his or her most resort win the same parish as well within lybertees / sentuaryes / as in all other places within the said parish and of the said ward. And also of all men & women under the said age of .xv. yeres having or taking profits of ony lands / tenements / rents / fees / annuytees offices / or corrodyes of ony yearly value / or having goods or cataylles moeuable to the value of .xl. s. or above / the names of women having husbands / & beggars living onely by almesse onely except. ¶ And over this that ye certify by your writing the yearly value of the said {pro}fyte as beforesaid / and the value of goods & cataylles moeuable of every person whose name that ye shall certify before us at the said day and place. And also which of them ben knights / & which of them ben alyens born and not made denyzens. And which ben labourers / handicrafty men / artyfycers / apprentyces / & servants not having lands or other profits to ony yearly value / nor goods moeuable to the value of .xl. s. And which ben house holders / according to the tenor & effect of thartycles here after specyfyed & declared as nigh as ye by your dyscrecyons & wisdom by search / demanding of questions / or otherwise can or may.  
FIrst ye shall certify by your said writing in one article by itself the names of every {per}sone being within the said ward / & dwelling or having his or her most resort to or in the said parish. And having profits of lands / tenntes / freehold / or copyhold / fees / annuytees / offices / or corrodyes to his or her own use to the yearly value of. xl.li. or above in ony place or places within this realm.  
¶ Also in an other article the names of every person being within the said ward / & dwelling or having his or her most resort in or to the said parish / and having profit of ony lands or ony other( the premisses) to his or her own use to the yearly value of .xx. li. or above / and under the said value of. xl.li. in ony place or places.  
¶ Also in an other article the names of every {per}sone being within the said ward / & dwelling or having his or her most resort in or to the said parish / & having profits of ony lands or of ony other( the premisses) to his or her own use to the yearly value of .x. li. or above and under the said yearly value of .xx. li. in ony place or places.

Women having husbands except.  
¶ Also in an other article the names of every person being within the said ward / & dwelling or having their most resort in or to the said parish taking wages / or other profits for wages to the yearly value of .xx. s. or above / and under the value of xl. s. being above the age of .xv. yeres. Women having husbands except.  
¶ Also in an other article the names of every person being within the same ward / & dwelling or having their most resort in or to the said parish / above the age of .xv. yeres taking wages or profits of wages under the value of .xx. s. by year. And also of every apprentyce and of every other person taking no wages nor profits for wages nor having nor taking ony profits of lands tenntes / annuytees / fees / offices / or corrodyes to his or her own use. Nor having goods nor cataylles to the value of .xl. s. Women having husbands( and beggars) alway except.  
¶ Also in an other article the names of all house holders / and the names of as many persons labourers / servants / artyfycers / apprentyces above the said age of .xv. yeres / taking wages of them / or taking no wages being within their houses. Women having husbands( and beggars) alway except.
